The service supports working-age adults who have moderate to severe affective and psychotic disorders. To be right for this job: The role of Co-occurring Conditions Community Care co-ordinator is to work in partnership with service users who are experiencing both mental health and substance use issues, developing servicer user care plans and recovery plans to enable them to lead fulfilling lives. The post holder will demonstrate an understanding of clinical interventions relevant to the locality teams protocols to include assessments, harm minimisation, motivational interviewing, psycho-education, risk management, relevant aspects of social inclusion, recovery, care plans and discharge planning, working closely with the local drug and alcohol services, carers, families and partner agencies to facilitate this You will have a degree or relevant professional registration as a Nurse, Occupational Therapist or Social worker or equivalent substantial working experience with the designated client group, You will be an active member of a dynamic, highly motivated, skilled and creative team You will be working in a supportive environment that values your supervision and professional development You will have a voice and role to play in service development. The ideal candidate will work at all times to promote the safety and the wellbeing of service users and their families/carers. The post holder will assess, plan and implement care, and provide specialist harm minimisation and reduction of substance use advice and carry out specialist clinical skills and procedures. They will provide clinical supervision to staff and students. The post holder will ensure that the locality NICE interventions and ethos are adhered to. You will be involved in the delivery of care-coordination to a specified caseload operating under the CPA framework, a daily duty service, ongoing risk management, care planning, support around benefits and housing, carers’ assessments, physical health care and a wide variety of therapeutic interventions for which you can access training and supervision. To undertake assessment of individuals and their families/carers accessing the service with complex health presentations, including those service users presenting with higher levels of risk. 2. To develop alongside the service user, their care plan. 3. To offer specialist professional assessment and advice where required. 4. To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers. 5. To regularly review risk factors and make changed to the management of them as necessary. 6. To work using assertive outreach approaches to engage the service user and their families. 7. To contribute to the maintenance and development of the Trust nursing strategy. 8. To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedures. 9.
